Financial Policy
We would like to take a moment to outline our financial policy to insure that there are no misunderstandings
so that we can all concentrate our full efforts towards you achieving optimal health.
Payment must be made in full when services are rendered. WE ARE NOT A MEDICARE PROVIDER.
Each visit subsequent to your initial consult, we will explain in advance all the fees for the services you will be scheduled for.
We do not bill insurance directly. We will complete a claim form for each visit, which you can use to submit to your insurance company for direct reimbursement. This CANNOT be done for Medicare.
The methods of payments which we accept include:
Personal checks ~ Cash ~ Visa ~ MasterCard ~ American Express ~ Discover
We require that you notify us of cancellations by 7:30am if you are scheduled for an IV or 3 business days if you are scheduled for a consultation to avoid being charged for your specific scheduled service.
This will allow us to fill that time slot.
Cancellation Fees (if we are not notified)
-IV Treatments – ½ of IV cost
-Office Visit / Consult and any other services provided by Dr. Waters – full price
In certain instances, these fees can be waived. Please call to work it out with the Office Manager.
